Vision Discount Program

A Vision Discount Program is a great investment in you and your family’s* well being. Ours is a comprehensive program covering both eyecare and cosmetic optical needs. Members save on eye exams, eyeglasses, contacts and surgical procedures; see below for details.

Our vision provider networks are comprehensive and include ophthalmologists (MDs), optometrists, independent optical centers and national chain locations. Plan members have access to over 45,000 vision providers in 21,000 eyecare locations nationwide.

There are no limits to the number of visits Program members and their families* may make to service providers. When making a purchase, one simply pays the bill, less the applicable discount. The process is really that simple. Payment must be made at the time of service in order to receive a discount.

How You Receive Your Vision Discounts:

• The first thing you do is locate a participating provider; (Click Here to locate a participating provider). You may also call Member Services at 1-877-851-0101 Monday through Friday from 8:00AM to 8:00PM Eastern time.
• Call the provider for an appointment. Verify that the provider participates in the program offered through EyeMed Vision Care. If your provider does not recognize your card or has questions, please call Member Services at 1-877-851-0101.
• When arriving at your provider's location, present your ID card and pay the provider at the time of service. This program does not make payments to providers. You are obligated to pay the provider or make payment arrangements at the time of service.
• Members are obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount plan organization.
• Ask the provider to call Member Services at 1-877-851-0101 with any questions.

Program members enjoy discounts as follows:

• Save 35% on a complete pair of eyeglasses.
• Savings on contact lenses (not including disposables) up to
  20%.
• Save $5 to $10 on eye exams.
• Save 15% on LASIK surgery at participating ophthamology locations.

• Shop at well-known providers such as:
  Pearle Vision, LensCrafters, JCPenney Optical, Sears Optical and Target Optical

• No limit on the number of times membership may be used during the year
